Patient Information

New patients making their first visit to see our providers are requested to arrive 10 minutes before their scheduled appointment, for the purpose of registration. Please have current insurance information and medication lists with you.

All patients are encouraged to call with any questions they have concerning their family’s medical problems. Messages will be taken and answered at the earliest opportunity.

Emergencies:
If you think you require emergency care during office hours, call the office. If the provider determines it to be an emergency, you will be advised to come directly to the office or the emergency room.

After-hours emergency calls are taken by our answering service. The provider on call should return your call promptly. In the event of delay, proceed to the emergency room for care. Please notify answering service if emergency.

You may have insurance, which will pay for services rendered. If your insurance has a co-pay, you must be prepared to pay this amount BEFORE THE PATIENT IS SEEN. We also require that you bring your current insurance card to the office AT EVERY VISIT. It must be understood that financial responsibility for the account rests with the patient. We cannot accept the responsibility for collecting your claim or negotiating a settlement on a disputed claim since we are not a party to your insurance contract.

All PPO and HMO patients must contact their PCP before going to another doctor, emergency room or a contracted urgent care facility.

Medicaid patients must have their Medicaid cards with them for every visit or they will not be seen

We accept most insurances.

If you require copies of medical and/or immunization records or referrals, please give the office 1 week notice. We will contact you when records are ready to be picked up.

Refill policy requires that refills be requested during regular office hours and be done with at least 24 hours notice. Refills are not done routinely on weekends or after hours.
